Gynecomastia is a male breast problem, enlargement of the male breast similar to the female breast. Breast tissue consists of adipose tissue and mammary glands (parenchyma) in men as in women. Breast enlargement can only be due to an increase in the amount of fat, an increase in the amount of parenchyma alone, or an increase in the amount of both fat and parenchyma. Which tissue has increased can be decided by breast ultrasonography. Breast enlargement in men can be seen due to genetics, hormonal changes, puberty, old age, excessive alcohol use, liver diseases or some drugs such as quinine.
In adolescence, slight enlargement, tenderness and pain in the breasts are considered normal, these findings are temporary. If it is permanent, it is considered as gynecomastia.
Breast enlargement (gynecomastia) in men is classified as 3 degrees from simple to severe.
Mild gynecomastia (grade 1); There is tissue increase without excess skin.
moderate gynecomastia (grade 2); Along with the tissue increase, some excess skin also occurred.
In advanced gynecomastia (grade 3), there is both an excessive increase in tissue and sagging of the breast, completely resembling a female breast.
Gynecomastia surgery can be performed under local anesthesia or general anesthesia, depending on whether the gynecomastia is mild or severe. If there is an increase in adipose tissue, it is sufficient to apply only liposuction. If the parenchyma that causes gynecomastia is increased, then it should be surgically removed. Sometimes it may be necessary to do both methods at the same time. If gynecomastia has become advanced and skin sagging has occurred, the excess skin will need to be removed as well as emptying the breast.
According to the characteristics of gynecomastia, if only liposuction is to be performed, the surgery is performed through two holes of 3 millimeters opened in the area towards the armpit on both sides of your breasts, and there is no scar on the breast. If the gynecomastia tissue is to be surgically removed, an incision is made that includes the lower half of the colored part around the nipple and only a very vague scar remains in this part, this scar does not cause visual problems. However, if there is significant skin sagging, if the breast has really grown to resemble a female breast, then gynecomastia surgery is performed similarly to breast reduction surgery in women and a scar remains in the operation area.
You will usually be discharged from the hospital the same day after surgery. After 3 days you are allowed to take a bath. You should use a gynecomastia corset or bandages that put pressure on the breasts for about 3 weeks.
